Electronic Medical Records (EMR) For individuals who provide phone or email contact information with ROC, and elect to schedule an appointment at ROC, your contact information is recorded in our EMR to provide appointment reminders by text, phone call and/or email. In addition, we utilize your email address to send a secure connection to complete online registration forms and access our secure online patient portal. Outcomes Tracking (DataBiologics) ROC believes that patient outcomes tracking is essential to the ethical delivery and advancement of regenerative orthopedics. To this end, we ask all patients receiving these services to participate in our outcomes tracking registry with DataBiologics by completing baseline and follow up surveys regarding pain and function. If you schedule a procedure, we utilize your phone number and /or email to send you these surveys. You may opt out of receiving survey texts and/or emails.
